Beyond Healthcare: Rethinking Women's Health in Uttarakhand

Abstract

Uttarakhand has witnessed considerable improvements in maternal and child health over the past two decades, particularly in institutional deliveries, antenatal care and maternal health indicators. However, these achievements have not eliminated persistent inequalities in women's access to healthcare, particularly in rural and hilly regions. This editorial examines the structural and social determinants that continue to shape women's health experiences across the state. Geographical isolation, difficult terrain, inadequate transportation, seasonal road disruptions, financial constraints and limited access to health facilities can restrict the timely utilisation of healthcare services. These challenges are further compounded by women's extensive responsibilities in agriculture, livestock management, childcare and household care, particularly in the context of male outmigration. The editorial argues that healthcare availability alone is insufficient to achieve equitable health outcomes. Education, nutrition, livelihoods, transportation, economic security and women's autonomy play an equally important role in determining whether healthcare can be accessed and sustained. A life-course and multisectoral approach is therefore essential for addressing persistent health inequities among women in mountain communities. Future health policies in Uttarakhand should move beyond service expansion and integrate social, economic and geographic realities into planning and implementation. Such an approach can help translate improvements in healthcare infrastructure into more equitable and sustainable health outcomes for women across the state.
